| FREE ATHLETIC PHYSICALS Portage Health Medical Group will be offering FREE athletic screenings to student athletes on Tuesday, June 18th from 10:00-11:30 a.m. Athletes should arrive prepared and must have a physical form (available in the principal's office) with completed health history. It MUST also be signed by a parent in order to participate. Bring eyeglasses or contact lenses for the vision test. You do NOT need to bring a urine sample. All athletes that will be in 6th-12th grade are encouraged to take advantage of these FREE screenings so that they are prepared for the 2013-14 school year. |
